What does a stripped lug nut look like?

Why do lug nuts get stripped in the first place?
Impact guns. A tire shop running a gun at full torque with a worn socket rounds the hex or over-torques the nut so hard that the next person cannot break it loose. Two-piece chrome-capped lug nuts — factory equipment on a lot of GM, Ford and Honda vehicles — also swell with age as moisture gets between the cap and the steel body.
Are swollen lug nuts a manufacturer defect?
